What are the six pieces of information needed for itemization of bills?
The number one hindrance to the timely publishing of a need is submitting insufficiently itemized bills. Please be sure that your bills include all of the following details:
1. Provider Information (name, address, and phone number of the provider).
2. Patient’s name.
3. Date(s) of service (this may be distinct from a statement date or payment date).
4. Description of services provided (for prescriptions, this includes the drug name, dosage, and quantity).
5. Charges for those services provided. (We are not looking for proof of any payments.)
6. Account number, if available.
